An Opportunity to Expand Partnerships to Promote Health Equity

The Patient Protection and Affordable Care Act authorized Community Transformation Grants to help local communities address racial and ethnic health disparities and reduce chronic diseases by promoting healthy living and tackling the social and economic causes of poor health. These grants are designed to encourage collaboration among diverse partners, including state or local government agencies, to promote healthier communities, and reduce racial and ethnic health disparities.

This call will explore the opportunities to become engaged in promoting health equity through these grants. The presenters will provide an overall understanding of grants and provide concrete examples of collborative projects to promote health equity at the local level. There will also be time for discussion and questions.
